Evaluation of Glutathione S-transferase M1 genotype and Heopterin as prognostic factors in p-Thalassemia Patients

This work was carried out to investigate the role of Neopterin and GSTMI polymorphism in prognostic F-thalassemia also to detect the correlation between GSTMI null genotype and appeamnce cardiac complications in p-thalassemia. Methods: This study was divided to three groups (group I: 20 healthy subjects, group II: 56 ftthalassemic patients, goup Itr: 16 B-thalassemic patients with cardiac complications). The human neopterin and cytokines were determined by ELISA metho4 while the measurement of human hs-CRP and Immunoglobulins were performed using nephelomofiy. GSTMI gsnotype was detected by PCR and cardiac complications were detsrmined by Echocardiogfaph. Results:There were statistically significant increases in Neorpterin, IL{, TNF-og IgA, IgG, IgG subclasses (1, 2,3 &, 4\ and hs-CRP levels, but statistically insignifrcant difference in IL-4 and IgM in Btlralassemic patients compared to normal subjects. Results showed no relation bstween GSTM1 null genotype &equency and pthalassemia or cardiac cornplications appeaftrnce. Also results showed that LV diastolic function was signifrcantly impaired in cases with GSTMI null genotype in group III. Conclusion: Neopterin can be used as a marker for complications of inflammation and infection in p-thalassemia. GSTM1 null genotype frequency has no role in betalhalassemia or cardiac complication appearance. GSTMI null gendype has a role in cardiac iron overload and impairing ofleft ventricular diastolic ftnction.
