Histopathological and immunohistochemical study of matrix metalloproteinase-2 and matrix metalloproteinase-9 in breastcarcinoma

Introduction: Breast cancer is the most frequent malignant tumor of women worldwide, with increasing incidence rates. The assessment of prognostic factors, in order to provide a prediction of outcome, has become an essential part of the histopathologist‘s role in the handling and histological reporting of invasive breast carcinomas. Stress on the need to understand the mechanism that regulates and facilitates the metastatic process. This step in metastasis of malignant cells requires the involvement of proteolytic enzymes which degrade the protein components of extracellular matrix. Matrix metalloproteinase 2 and matrix metalloproteinase 9 are members of degrading enzymes involved in cancer development, invasion and metastasis. Material & methods: Sixty cases of breast cancer with positive and negative lymph node were collected randomly retrogressiveas tissue sections and cell block prepared material. The cases were stained by Haematoxylin and Eosin, MMP2 and MMP9 immunostaining to assess their usefuleness in growth, invasion and metastasis.Results: The majority of cases were presented in the age group 51-60 years, representing (39%) of the total count of cases. This study revealed that the most common type was invasive duct carcinoma NOS, representing 70% of cases. The MMP2 and MMP9 represent 85% and 90% in tumor cells respectively. Their presence in peritumoral stroma is in the ratio of 60% and 64% respectively. Concerning the nodal status, byapplication of TNM staging system, the majority of cases (82 %) showed positive nodal status. MMP2 and MMP9 were significantly elevated in malignant tissues of patient with lymph node metastasis as compared to malignant tissues of patients without lymph node metastasis (p=0.029 and p=0.048 respectively). The activity of MMP2 and MMP9 increase with advanced clinical staging (p value ≤ 0.001).
